We welcome all new patients. It is our goal to make your first visit as convenient and comfortable as possible. To help us achieve this goal, arrive at least 30-minutes before your scheduled appointment. This gives you time to provide us with some necessary information.

Remember to bring:

• A complete list of medications you are taking or the actual medication bottles.
• Any vitamins, herbal supplements or over-the-counter medication you are taking because the medications can have a reaction to medications we may prescribe.
• Immunization records and a copy of your previous records, if you are transferring to us from another office.
• Your insurance card - to comply with insurance company requirements, insurance cards must be shown at each and every visit.
• Completed paperwork mailed to you.
• Referrals, if applicable.
• Comfortable clothing if having a treadmill test.
