Our office hours and guidelines for scheduling and canceling appointments.

Office Hours
Primary Care - Monday - Friday, 8:00 a.m. - 5:00 p.m.
Dermatology - Monday - Friday, 8:00 a.m. - 5:00 p.m.

As a courtesy to all of our patients, call ahead to schedule appointments and arrive on time for your appointment so that we may keep waiting time to a minimum. New patients should arrive at least 15-minutes before their scheduled appointment to complete necessary paperwork.  (Click here to download a copy of the patient registration form)

Confirmation and Cancellation
As a courtesy to our patients, we make every effort to remind our patients of their appointments with a telephone call one day prior to your scheduled appointment. In the future, patients who provide cell phone numbers and/or email addresses may be contacted by this means as well. If you fail to give us notice of your cancellation, the following policy is enforced:

First missed appointment: No charge
Second missed appointment: $25.00 charge
Third missed appointment: $25.00 charge and possible dismissal from Premier Medical Associates

Patients who are late for appointments may be asked to see another provider in the office or if more than 15-minutes late, may be asked to reschedule their appointment. Patients are asked to notify the office at (336) 774-3740 if they are going to be late for their scheduled appointment time.

Walk-in appointments
Our providers attempt to accomodate walk-in patients after patients with scheduled appointments have been seen, based on their medical urgency. To reduce the possibility of a prolonged wait, patients needing same day or urgent visits should call the office to schedule an urgent visit appointment.

After hours care
Providers at Premier Medical Associates provide 24-hour on call service in partnership with Call-A-Nurse. Patients with after hours, weekend and holiday medical issues should call our office at (336) 774-3740 to receive medical instructions. Patients experiencing an urgent or emergency medical issue should call 911 or go to their nearest emergency medical provider. 

Completion of forms
A nominal charge is required for filling out patient forms. This needs to be paid when the form is dropped off for completion. Some patients may be asked to schedule an office visit to fill out certain, more complex medical forms. Patients should provide seven (7) business days for forms to be completed.

Copies of medical records
Medical records are available upon signed request with payment of a nominal fee for copying and a signed authorization from the patient to release the records. Patients should provide seven (7) business days for records to be copied and the request to be completed.
