Insurance card, payment and a list
of current prescriptions, as well as over-the-counter medications, any films and reports of X-rays, MRIs or CT scans that you had recently, if necessary.
An emergency generally involves an illness that is currently or potentially life threatening. Although this is not a complete list, here are examples of true emergency situations: chest pain, stroke symptoms, trouble breathing, sudden, severe pain, such as a headache or stomachache, head or back injuries, bleeding or vomiting that won't stop, loss of consciousness, poisoning, major burns and cuts, choking, an obviously crooked extremity after a fall or accident. Individuals experiencing any of these conditions should go to the nearest emergency room or call 911.

support a patient's rights to make decisions about their medical care and treatment, especially in situations where they cannot make their wishes known. We have information available about advance directives, living wills and durable power of attorney. Ask our receptionist for copies of these important documents.
If you feel the nature of the illness or injury is life-threatening, you
should seek medical attention immediately by calling 911. For other emergencies
call 718-1500, identify your call as an emergency and be prepared to explain the
situation. During business hours, your call is routed to your primary care
physician. After hours, on weekends and holidays, your call is routed to
our triage service. The patient receives advice to stabilize the condition
or receive direction to contact your primary care physician during regular
business hours, come to WSHC's Convenient Care Clinic or proceed to an emergency
department.
When you need help in a hurry, the Convenient Care Clinic is ready. When you are sick and need to see a doctor during regular office hours first call the office of your regular Winston-Salem Health Care physician. Our physicians have opened their schedules to have openings each day to see patients with immediate needs. If available appointments with your physician have been filled, you may come to the Convenient Care Clinic at a time that best meets your needs.
At Winston-Salem Health Care, hundreds of healthcare professionals including physicians, mid-level providers, radiologists, pathologists, clerical and support staff work with patients behind the scenes to provide our patients and their families with the care and services they deserve. In addition, Winston-Salem Health Care offers a full service pharmacy adjacent to our main building, radiology services including x-ray, low-dose mammography and ultrasound, a new spiral CT Scanner producing detailed images for the early detection of cancer and other diseases, a full-service laboratory a shuttle service and an ATM machine.
Located at the rear of the main parking area is a phone and parking area
for those that wish to use our shuttle service. Just pick up the phone and a
central receptionist responds and immediatley sends the shuttle to pick you up
and take you where you need to go on the WSHC premises.
Our providers work with the Inpatient Physicians of Forsyth (IPOF), a team
of very qualified hospital specialists at Forsyth Medical Center. IPOF is a
multi-specialty team of physicians who admits and cares for our patients during
their hospital stay. Our providers are in communication with the inpatient
physicians regarding treatment plans and questions regarding care and discharge.
We have complete trust and confidence in the expertise and ability of these
physicians to offer responsive, quality care to our patients throughout their
hospital visit.
Choosing a primary care doctor is a great way to maintain exceptional health throughout the course of your life. In addition to treating acute illnesses, your primary care physician can partner with you to meet your long-term health goals and needs. Over time, this relationship can translate into continued health and enhanced quality of life.
To determine whether you need a referral from us before seeing a medical
specialist, the terms of your insurance plan. Often, this information is listed
on the insurance card or can be obtained by calling the insurance company.
Our medical providers can refer you to a specialist if your health situation
requires it or if your health insurance company requires a referral prior to
visiting a specialist.
To release records to a specialist or another practice, we need your written permission. We have forms in the office available for that purpose. The process of transferring records requires approximately two weeks for completion.
