Medical Resource Management - Nurse Educator, Full time

This position provides identification, assessment and
management of high risk individuals with diabetes using disease management
concepts and evidence based guidelines. Responsibilities include
completing disease specific assessment and creating a plan of care that promotes
self management of diabetes. This is supported by teaching individuals 1-1
and in group settings as well as utilizing telephonic coaching and medication
management skills. The position promotes effective self management by
further support to families and care givers and serves as a resource to
physicians and clinical staff. Major Job Functions: Monitors care
for high risk diabetes individuals and their families consistent with evidence
